About this service
The community outreach assessment support and treatment team provides services for persons with both intellectual disability and also mental illness. These services include:
Assessments including a detailed report and recommendations
Short-term treatment and support with a view to discharge client back to his/her family doctor
A plan for leaving the hospital and follow up after patients leave the hospital
Working in collaboration with family physicians and other community health care providers to facilitate the recovery of individual clients.
