About this service
Person Directed Planning is a process of imagining what a persons idea ofa positive and possible future looks likeand thenbringing together the resources and connections that will help them to achieve those goals.
The Person-Directed PlanningProgram works with people to discover and work towards their personal vision of a positive and meaningful life. Participants work with a Person-Directed Planning Facilitator to identify 1-3 goals that reflect their values. Together, the goals are broken down into manageable steps that can be worked on individually, with community resources and/or other supports.
Participants may create goals one or more of the following areas; Housing, Recreation and Leisure, Employment and Volunteering, Community Exploration, Self-Regulation, Social Connection and/or Life Skills
