About this organization
The Community care program is an initiative of Eastern Healths Long Term Care Program. It is a community based supportive housing program for individuals who require support as a result of a severe and persistent mental illness.
The Community Care Program provides residential, psychiatric, case management and therapeutic recreation services to voluntary residents. Community Care Homes are privately owned and operated with the majority being located in Conception Bay area.
