About this service
Flexible Assertive Community Treatment (F.A.C.T. Team) provides care to people with serious mental illness who are currently residing in the community. Such F.A.C.T. clients have severe and persistent mental illnesses with many limitations in their overall functioning. The FACT model has been proven to effectively reduce hospital stays and help clients with serious mental illness stay connected to available services.
