Cost
You pay a set fee for each dental appointment. You will not be refused service based on your inability to pay. Your fee will depend on your net family income and the number of dependents you have, such as children or parents who rely on you for care. You must provide us with a copy of your most recent Revenue Canada Notice of Assessment (Tax Form T451) to help determine your dental costs.
