HAND - ERIK

ERIK is a Kit with a simple series of forms folded into a plastic folder that is placed on the homeowners fridge with magnets. This Kit is very beneficial as it includes pertinent health information, next-of-kin, medication list, and a health care directive. A red sticker is placed on the home's front door to notify EMS that a Kit is completed. If paramedics are called into your home, they will look on the fridge for the Kit to provide them with information they may need.

The Health Care Directive provides information one's wishes when they are unable, such as: treatment instructions. Also, the circumstances in which one does or does not wish to receive set out clearly and precisely.

Please remember instructions can only be carried out if they are set out clearly and precisely. Also, proxies can be designated to carry out decisions if one is unable to do so. It is recommended that everyone speak to their family physician before completing the Directive.
