About this service
Services for Seniors has an Outreach Program. We aim to continue to help seniors stay in their homes longer by reducing isolation and increasing their mental and physical health and safety.
Refer yourself or a loved one to our Outreach Program to reduce isolation and loneliness! Once the referral is made, the Resource Coordinator will contact the referred participant to complete an assessment.
