About this service
a transitional program as students prepare to leave the school setting anticipating entering into the adult services day program after 21 years of age. Alternative academic/life skills programming using close community-school-home partnerships.
This program provides an individualized alternative setting for individual education plan (IEP) accomplishment. Students of the Making Connections Program benefit from a small facility with small staff to student ratio.
Making Connections participants are able to work on their academic material as well as establish a stronger base of life/independent living skills and pre-employment skill development.
Each participant has a complete plan of care focused on short and long term goals with strategies implemented to achieve them while complimenting their IEP.
At age 21 students graduate from the program and have the option to continue with their day to day program by entering into the Network 4 Change Healthy Independence Program.
Who can use it?
Students between the ages of 13-21 referred by School Division student services teams & admin
