About this service
Our intensive case management team (ICM) accepts referrals for young people requiring more intensive and specialized mental health and substance use support. All young people receiving ICM services are connected with a case manager and psychiatrist specializing in youth/young adult mental health and substance use.
The ICM team members check-in with youth regularly on an out-reach basis to see what is going on in their life, develop care plans, and link them with the centres resources and community services to support their treatment and recovery goals.
