About this service
Offers health care and support services to adults with acute, chronic, palliative, or rehabilitative health care needs, and supports the transition from hospital release to home life; intake is required to assess eligibility. Home Support provides assistance with medications, bathing, and dressing for clients who are ill or recovering from illness or surgery, have physical disabilities, require personal assistance or respite for a caregiver, or need end-of-life care. Nursing Services may include assessment, education, counselling, and medical and surgical care at home. Physiotherapy and Occupational Therapy referrals are for clients who have a physical disability, are recovering from surgery or hospital stay, or who need rehabilitation or therapy to stay at home. Also provides assessment for access to Adult Day Services (day respite, health monitoring, therapeutic and recreational activities, and community bathing programs).
