About this service
The Transitions Program provides Withdrawal Managementand Supportive Recoveryfor youth ages 13-19 who would like to take a break from substances. The programstake place insupportive, family environments we call Care Homes. Participants will have their own room, be provided with transportation, and will share meals, chores, and maybe even new hobbies. While staying at a Care Home, participants will attend the Pathways to WellnessProgram. This program will include other youth, will be guided by our fun and energetic Group Facilitators, and will include activities such as recreation, cooking, art, sports, cultural events, and community adventures. At Transitions, it is important that all participants and staff feel respected and safe. Allcultural backgrounds, spiritual beliefs, genderidentities,andsexual orientations are welcomed and celebrated.
