About this service

L2T goals are to engage and link newly diagnosed individuals to treatment and/or support individuals return to treatment. Our program incorporates a case management, psycho-social client-centred approach to support and engage individuals to access HIV/HCV treatment support and care. We use evidence based interventions to promote and monitor treatment adherence. As treatment regimens are complex and the impact on health and wellness significant, L2T welcomes community partners involvement for a holistic, connected, care team approach and in cases where there are physician referrals, coordination support for the referred person.
Eligibility
- Newly diagnosed with HIV and/or HCV
- Re-engagement into care for HIV/HCV positive individuals
- HIV and/or HCV positive individuals who would benefit from strengthening their engagement into care
