L2T- Linked to Treatment As part of the Provinces STOP HIV/AIDS initiative, CVIHRS through Island Health is now able to offer care, connection and coordination for eligible and referred individuals with HIV/HCV through a coordinated, integrated multidisciplinary case management service. This initiative is part of a province wide effort to reduce transmission of HIV and HCV and engage HIV/HCV positive individuals into treatment. L2T goals are to engage and link newly diagnosed individuals to treatment and/or support individuals return to treatment. Our program incorporates a case management, psycho-social client-centred approach to support and engage individuals to access HIV/HCV treatment support and care. The program uses evidence based interventions to promote and monitor treatment adherence. As treatment regimens are complex and the impact on health and wellness significant, L2T welcomes community partners involvement for a holistic, connected, care team approach and in cases where there are physician referrals, coordination support for the referred person.
