About this service
Our Health and Wellness program provides clients who fit our mandate with individualized services and supports to address needs related to health, housing, and income. Our Health and Wellness Worker conducts an initial intake to assess each individuals unique needs, and works with the client to develop a work plan to meet those needs. Work plans are a collaborative effort, and it is expected that clients actively participate in their individual work plans. When we are unable to address a specific need we do our best to refer clients to other appropriate services within the community.
