About this service
In home client support to bridge the gap between acute and community services for medically complex/frail elderly
patients who are not known to Home Health.
Short term services include:
- Home Care Nursing
- Home Support Worker
- Respiratory Therapist
- Social Worker
- Occupational Therapist
- Physiotherapist
QRT provides referrals to Home Health and other community based services that may be required beyond the normal service period of 1-5 days; maximum service length is 2 weeks for complex discharges.
