About this service
A partnership between ASK Wellness Society and Provincial Health Services Authority (PHSA), this program seeks to identify the gap in services faced by individuals transitioning from corrections back into community. Initially designed to ensure individuals transferring out of corrections had access to opiate replacement therapy, the programs scope has consistently grown, based on the needs of the community. The CTT program focuses on comprehensive transition planning, involving access to housing, ensuring adequate physical and mental health, and providing supports.
