About this service
We believe that home, with appropriate supports, is the best place to continue your recovery from illness and injury, manage chronic conditions and live out final days. Evidence shows that returning home from hospital as soon as possible with the supports you need gives you the best chance to regain your strength and independence, even if you are not yet totally able to care 100 per cent for yourself. Home may be any living space, whether house, apartment, condo or shared accommodation.
Sometimes a large space such as a house becomes unmanageable as frailty increases. We recommend downsizing before considering more drastic forms of housing such as long term care. It may also be a good idea to retrofit the living space with assistance devices to enhance safety. A personal alarm service such as Lifeline would also be beneficial.
