About this service
This program is for individuals who are taking blood clotting medications (Warfarin, Eliquis, Xarelto, etc). Our clinical pharmacists will:
- Educate individuals on their medication and the signs and symptoms of bleeding and clotting
- Advise individuals when they need to change their dosage
- Provide individuals with lab requisitions for INR or other relevant blood work, and medication refills
- Monitor results and follow up with individuals as needed
- Act as a resource for individuals, physicians and dentists in regards to anticoagulation
