Capacity assessment
Appointments at the Geriatric Clinic involve:
Assessing patient health and dealing with any issues.
Developing a treatment plan to help patients be healthy and independent.
Discussing treatment plans with patients and their family/caregiver.
Providing information regarding health promotion, disease and injury prevention for the medically complex and those with chronic disease.
Making contact with any required community services.
Partnering with family physician to manage ongoing care.
A family member/friend must be in attendance at the appointment.
