About this service
The Diabetic Wellness Clinic is a physician-led program, supported by a multidisciplinary team of PCN health care professionals. The clinic will provide planned visits, coordinated care, counselling and follow-up for patients with diabetes. The program follows the current Canadian Diabetes guidelines.
Diabetes is a chronic disease characterized by higher-than-normal blood glucose levels which can be caused by lack of or resistance to insulin, excess weight, family history, age, other diseases, or a combination of these.
