About this service
Continuing Care (Long Term Care) supports individuals with more complex health needs whose care
cannot be safely provided in their own home, or in designated supportive living. Long term care facilities are sometimes called nursing homes.
Continuing care services are available through Alberta Health Services once you have been assessed by a Case Manager. If you do not have a Case Manager, call Health Link at 811.
