About this service
The Senior Services Outreach Coordinator provides support, information, and referrals to enable seniors to remain as independent as possible while maintaining dignity and optimum quality of life.
The focus is on strengthening the well-being and improving the quality of life for the Town of Sylvan Lake & Summer Villages seniors.
The FCSS Senior Services Outreach Coordinator will meet with seniors and/or their families either in the office or in the senior's home to provide referral's such as but not limited to:
- Financial Supports
- Mental Health
- Grief & Loss Resources
- Social/Recreational
- Mobility Concerns
- Transportation/Accessibility in the Community
- Senior's Housing Options
- Caregiver Support
- Support Groups
- and MORE!
The FCSS Senior Services Outreach Coordinator can also provide assistance with filling out forms
