About this service
Pathways to Housing targets chronically homeless persons who are disabled by serious mental illness and addiction as well as health and social issues. This program is similar to the Amethyst House model with the addition of Street Connect, a multidisciplinary AHS clinical mental health team. CMHA provides a Peer Support Recovery Specialist for additional on-site support.
To Access the Program
If you are homeless and looking for housing, we can connect you to a Coordinated Entry Team. They will ask you questions to help determine what you need. Your information will be sent to a team of housing programs who decide which program is the best fit. It may not be with CMHA. If all the programs are full, you will be put on a wait list.
