About this service
Having a new or existing chronic condition can be a bit scary and confusing. Whether it is a chronic condition such as diabetes, high blood pressure or high cholesterol, your Family Physician and our PCN team work with you to help you improve your health.
We are here to help you manage your chronic condition based on the goals you set and want to achieve. For example, we can support you with:
- losing weight and safely increasing your physical activity
- developing a personalized approach to assist you in meeting your goals for tobacco reduction. This may include meeting with members of our PCN team as well as additional community resources.
- using strategies to make healthy lifestyles choices and with staying motivated
