About this service
The Transition to Independence Program works in collaboration with the youth and their Children's Services Case Worker to provide flexible and responsive transitions for youth 16-24-years-old with Child Intervention status to adult services and/or independence. The youth may be in transition from residential care, foster care, a parental home, incarceration, hospitalization, or homelessness; lack sufficient family support and/or community resources to stabilize the youth in independent living; and have presenting issues including, but not limited to, history of abuse or neglect; physical and verbal aggression; sexually inappropriate behaviour; poor family relationships and a history of parent-child conflict; school difficulties; legal issues; substance use issues and addiction; self-harming behaviour and suicide ideation; concurrent disorder; poverty; and pregnancy and/or parenting.
