About this service
This inpatient program specializes in the inter-disciplinary assessment and treatment of seniors with complex medical problems.
The provision of integrated health services is directed toward elderly patients experiencing potentially reversible medical problems which are contributing to a physical, psychological, or social decline in function. The program is restorative and rehabilitative in its focus and endeavors to facilitate a patients ability to return to their own home or, when necessary, recommend alternative living options.

- makes referrals to supports and resources as needed when discharge happens
