About this organization
Our programs play a key role in allowing medically stable individuals to remain living in the community as long as possible by optimizing their level of physical, cognitive, social, and emotional function.
Appropriate for adults who are:
- Able to regularly attend and participate in the program
- Able to function appropriately in a group setting
- At risk of losing their ability to manage independently in the community but wish to continue to live at home (and can do so safely)
- With a history of increased health care utilization
- With stable medical conditions
- With signs and symptoms of cognitive impairment
- Whose caregiver is in need of respite
Each day is intentionally designed with the preferences, interests and needs of each member in mind. The schedule is predictable to help with memory retention, yet flexible to accommodate individual preference and endurance. The content varies to provide interesting and stimulating programming.
