The Unattached Complex Care Clinic supports patients in the Grande Prairie area who are being discharged from the Grande Prairie Regional Hospital and do not have a family physician. The Grande Prairie PCN and Alberta Health Services have collaborated to establish the Unattached Complex Care Clinic, which aims to help the transition of patients back to community care by temporarily managing medical needs, stabilizing patients, and decreasing the risk of further hospital readmissions.
