About this service
Offers education and support for a healthier, more active lifestyle to reduce the risk of future heart problems. It offers teaching and support for patients and families to understand how best to lead an active and healthy lifestyle through focusing on diet, supervised exercise sessions, and other cardiovascular risk reduction activities. It provides teaching and follow-up to people with a history of heart problems, including myocardial infarction (MI), acute coronary syndrome (ACS), coronary artery disease (CAD), arrhythmias, and heart failure. It also provides teaching and follow-up to people waiting for an angiogram/angioplasty, pacemaker, cardiac bypass, or valve surgery.
