The Grande Prairie PCN - Persistent Pain Program is a program that offers high-quality, comprehensive, and client-centered care for individuals suffering from persistent pain in Grande Prairie, Fairview, Grimshaw, and surrounding areas. The care is delivered through a multidisciplinary team composed of Nurse Case Managers, Physicians, Nurse Practitioners, Physiotherapists, Kinesiologists, Social Workers, Dietitians, and Mental Health Therapists who work collaboratively with primary care providers. Patients are provided with individual treatment plans aimed at self-management of chronic pain and enhancing the quality of life, with regular reassessment and adjustment of treatment being a key component. The program is delivered through various methods like individual appointments, group sessions, workshops, and classes. Referrals can only be made by PCN physicians.
