About this service
When a loved one has been discharged from a medical facility, the last thing you want is for them to be readmitted. Unfortunately, roughly one-in-five seniors are readmitted to the hospital within the first 30 days of being discharged. At Senior Home Care by Angels we work to reduce these high rates of hospital readmission by offering transitional care services.
