About this service
Supports families with children with complex physical medical needs to help them remain safely in their homes, schools, and communities.
Based on assessed unmet needs, Children's HC provides case management along with health and developmental services and supports to children / families in their homes or community setting including:
- transition from hospital to home
- acute health needs - client may be seen in HC Ambulatory Clinic at Stollery
- long term health needs
- end of life support and care
Partners with Education and Disability Services under the Edmonton Regional Collaborative Service Delivery (ERCSD) framework to support school personnel and families with children with complex medical needs to remain safely in their community.
Who can use it?
Children and youth, from birth to 18 years of age
