About this service
Journeys supports older adults and seniors with developmental disabilities and mental health diagnosis. The focus of this program is to provide opportunities to create and foster meaningful relationships, proactive health care, assistance in finding and securing appropriate housing, facilitating leisure opportunities and offering educational services to promote a healthy lifestyle. The Journeys program provides support based on the individual clients needs and wants. We can provide anywhere from 5 hours of day support per week to 24 hour staffed residence.
Referrals can be made by the individual, family, health care provider, PDD or supporting community members.
