About this organization
Support of children and youth aged 6-20 with dual diagnoses including at least one complex mental health concern. Through a family-centred approach, CONeX (Collaboration Outreach Navigation and Exchange of Information) works alongside families and caregivers to connect to the best supports for their child / youth across home, school and community settings. Families involved with our team are provided comprehensive, integrated, time limited case management and collaborative support across systems.
