Adult Day Program

Our programs play a key role in allowing medically stable individuals to remain living in the community as long as possible by optimizing their level of physical, cognitive, social, and emotional function. Our programs are now being offered in a hybrid fashion with virtual and in person programming. Adult Day programs are appropriate for adults who are: able to regularly attend and participate in the program able to function appropriately in a group setting at risk of losing their ability to manage independently in the community but wish to continue to live at home (and can do so safely) with a history of increased health care utilization with stable medical conditions with signs and symptoms of cognitive impairment whose caregiver is in need of respite What can I expect? Each day is intentionally designed with the preferences, interests and needs of each member in mind. The schedule is predictable to help with memory retention, yet flexible to accommodate individual preference and endurance. The content varies to provide interesting and stimulating programming. Everyday living activities are incorporated throughout the day to feel like home

Home Care clients (with a valid Alberta Health Care Card) who live in a community setting are eligible to participate in our Adult Day Programs. Those who live in supportive living, long term care, or are receiving funding from Persons with Developmental Disabilities are not able to attend the Adult Day Program. Referral Needed Ask your case manager for a referral. If you do not have a case manager, call the Continuing Care Access line 1-855-371-4122 for a referral. Family, friends, neighbors or health professional can also call on your behalf. For any questions or to discuss whether the program is an option for you or family member, please call. Service Access You can self-refer by calling CCA and request an assessment for home care.

Adult Day Program is open Tuesday 9:00 am - 4:00 pm, Wednesday 9:00 am - 4:00 pm, Thursday 9:00 am - 4:00 pm.
