About this service
Support for pregnant and parenting women and teens (infants 0-6months) facing a variety of health and social challenges.
Healthy Moms Healthy Babies supports pregnant and parenting women and teens (infants 0-6months) who face challenges with isolation, finances, physical or emotional health, healthy nutrition, family conflict and / or addictions. Offers these services:
- group support with nutrtition and prenatal health education
- one to one visits with a nurse, dietitian, and / or outreach worker
- food vouchers and prenatal vitamins as needed
- referrals to healthcare providers or community agencies as needed

Pregnant women and teens or with infants 0-6 months of age
